This survey is being conducted by the Allegheny County Health Department. It is open to people who live in Allegheny County who are 18 years of age or older. Results from this survey will help the Department and community partner organizations determine health priorities to focus on for the next 3-5 years.
ACHD is seeking public participation in its Community Health Assessment Survey supplement, which is designed to better understand the complex impacts 2020 had on community residents. It is open to people who live in Allegheny County who are 18 years of age or older. The survey has 23 required questions and 5 optional questions. It will take between 5 and 10 minutes to complete. After survey completion, participants may enter a raffle for a $50 Giant Eagle gift card.
